Thank you for your referrals to our Family Planning division. We see patients in both Complex Contraception Clinic and in our pregnancy termination service, the Options Center. To expedite appointment scheduling and to ensure that your patient receives the best care possible, please complete the appropriate referral form below and submit the form, along with the requested supporting documents, to the fax number listed.

We will contact your patients directly to schedule the appointment and will send our visit summary to you as you request on the referral sheet.
